Cedars-Sinai Parkinson's Program:
Coordinating Patient Care

We are strengthening coordinated patient care by supporting the following outcomes:

  • Form and deploy new clinical care & research team for rollout of Kneu Health Program.
  • 300 patients experience optimized care through Kneu Health Program rollout.
  • Approximately 1000 patients experience improved continuity of care through new hospital dashboard.​

Providing Hope for Parkinson’s Disease Patients

Our support is strengthening integrated Parkinson’s care and accelerating treatment innovation through the following outcomes:

  • Advance clinical research to enhance treatment outcomes and develop new therapies for Parkinson’s disease.
  • Deploy innovative care solutions, including deep brain stimulation, AI-enabled wearables, pharmacogenomic testing, and remote monitoring technologies.
  • Build multidisciplinary care models, expand infrastructure, and strengthen patient navigation and support systems.
  • Scale community outreach and education to increase awareness, access to care, and participation in clinical trials.

Why it matters

There are many whose health conditions are left on the margins of treatment conversations and developments, yet they carry on living with great courage and care. We want to ensure that such persons and conditions are not left unnoticed. More can be done to meet their critical care gaps and we want to leverage the best of modern technology and innovation to do so.

How it’s unfolding

At Dana-Farber Cancer Institute, our support has enabled one of the world’s most comprehensive infrastructures for Stage IV breast cancer, enrolling over 4,000 patients, and created an AI-powered data repository from 20,000+ patients across all stages of breast cancer, including those with metastatic disease.

At Cedars-Sinai, the Kneu Health Program is transforming Parkinson’s care through a digital platform that bridges hospital and home. The program aims to reach 300 patients directly and improve continuity of care for 1,000 more, ensuring every patient’s journey is better coordinated and more compassionate.

At Baptist Health Miami Neuroscience Institute, our support is advancing more integrated Parkinson’s care. Accelerating research, enabling innovative treatments, and strengthening multidisciplinary delivery. Through improved coordination and expanded access to emerging therapies, patients experience more seamless care and better outcomes.
